Required Sports Medicine Forms

If you intend to participate in varsity athletics at Wells College in 2023-24, you are required to complete a checklist of items before you are cleared to participate as a full member of the team in practices and games. All medical forms must be completed well before the first day of practice - the documents must be approved by both the Community Health Center and the Sports Medicine staff and cannot simply be returned immediately before the first practice.

A list of all of the required documentation reads as follows:

FOR FRESHMEN, TRANSFER STUDENTS, EXISTING WELLS STUDENTS NEW TO VARSITY ATHLETIC

1. Wells Express Medical Forms 2023-24 - A comprehensive medical form created at Wells.
2. SportsWare Online Information Input - Members of the Wells Sports Medicine staff will utilize Injury Tracking Software that will assist in the diagnosis, treatment and management of injuries.This information must be entered by student-athletes and not their parents/guardians. To sign up, click "Join Sportsware" and type the School ID in the box (School ID: wellsexpress).

The Athletics Physical Examination Form must be completed and returned to the address below. The documents will be processed by the Community Health Center and then sent to the Sports Medicine staff for further approval.

FOR RETURNING STUDENT-ATHLETES

1. Returning Student-Athlete Health Form - This form is designed merely to update medical information on a year-to-year basis for student-athletes who have previously participated in varsity athletics at Wells.
2. SportsWare Online Information Input - Returning student-athletes must log in to the SportsWare online account that has been previously set up to verify that all information (including insurance coverage information) is current and correct. Wells College requires that a copy of the front and back of the insurance card is uploaded into the SportsWare account.
3. Returning Student-Athlete Sickle Cell Form - Returning student-athletes must complete the sickle cell form each season.
